The Road Home was founded in 1923 and has been a leader in the endeavor to end homelessness for over 100 years. The Case Manager will work as part of our Wendell and Palmer Court case management team to provide direct case management and supportive services to individuals at the Wendell Apartments and individuals and families at Palmer Court who have moved out of homelessness and are living in permanent supportive housing at the Wendell and Palmer Court.
